Provider registration information

Register as an HBF provider quickly and easily.

This section contains details of documentation required, along with the relevant forms that can be downloaded for completion.
 
For any questions about HBF Provider Registration you can contact us in the following ways:

• Acupuncture

  To be eligible for HBF Registration as an Acupuncture Provider, you must practice solely and exclusively in private practice, as stated in HBF’s 'Fund Rules' (reference: e3.7)

  Please note

  You will need to complete the application form below and return it to HBF by mail - GPO Box C101, Perth, WA 6809 - or fax it to (08) 9265 6282.

  In addition to the above you will need to meet HBF’s qualification criteria and belong to an HBF Approved Association. View current HBF approved Associations and qualification criteria.

  NB: Providers practicing in Victoria must be registered with the Chinese Medicine Registration Board of Victoria; there is no requirement to belong to an HBF Approved Association.

  Your membership of an HBF approved association for each profession is a requirement of eligibility for HBF Registration.  It is your responsibility to ensure that your Professional Indemnity Insurance and First Aid Certificate plus your Association membership are always current.

  How to register with HBF

  Provider Registration Form

• Chiropractors

  To be eligible for HBF Registration as a Chiropractor, you must practice solely and exclusively in private practice, as stated in HBF’s 'Fund Rules' (reference: e3.7)

  Please note

  You will need to complete the application form below and return it to HBF by mail - GPO Box C101, Perth, WA 6809 - or fax it to (08) 9265 6282.

  You will also need to provide a copy of your registration with Medicare Australia, for each location you wish to register with HBF.

  If you are registered to use electronic claiming (HICAPS/iSoft) and you are a WA provider, you will be registered as an HBF provider.

• Clinical Psychologists

  How to register with HBF

  You will need to complete the application form below and return it to HBF by mail - GPO Box C101, Perth, WA 6809 - or fax it to (08) 9265 6282.

  If you are operating within WA

  To be eligible for HBF Registration as a Clinical Psychologist practising within Western Australia, you must practice solely and exclusively in private practice, as stated in HBF’s 'Fund Rules' (reference: e3.7)

  You will also need to provide the following:

  • A copy of your registration with Medicare Australia, for each location you wish to register with HBF.

  If you are operating outside WA

  To be eligible for HBF Registration as a Clinical Psychologist practising outside of Western Australia, you must practice solely and exclusively in private practice, as stated in HBF’s 'Fund Rules' (reference: e3.7)

  You will also need to provide a copy of your registration with Medicare Australia, for each location you wish to register with HBF.

  We also require:

  • Evidence of completion of an approved Australian Psychological Society Masters course in Clinical Psychology

  or

  • Full membership to the Australian Psychological Society College of Clinical Psychologists

  or

  • Eligibility to become a full member of the Australian Psychological Society College of Clinical Psychologists (must submit academic transcripts and Statutory Declaration declaring the provider has completed required supervised clinical practise)

  How to register with HBF

  Provider Registration Form

• Dental Practitioners

  To be eligible for HBF Registration as a Dental Practitioner, you must practice solely and exclusively in private practice, as stated in HBF’s 'Fund Rules' (reference: e3.7)

  Please note

  You will need to provide a copy of your registration with Medicare Australia, for each location you wish to register with HBF.

  or

  If you are registered to use electronic claiming (HICAPS/iSoft) you will be registered as an HBF provider.

  HBF dental arrangements*

  HBF’s participating dental arrangements are designed to improve dental benefits for your HBF patients. As a participating dental provider, you continue to charge HBF members fees up to your individual fee schedule agreed with HBF.

  This schedule is based on the fees you currently charge to HBF members and means that HBF members can get between 60% and 80% back subject to their level of cover, waiting periods and maximums.

  If you are interested in becoming a HBF participating dentist, please see the Dental Arrangements Brochure below for further information.

  *The HBF Dental Arrangements are only available in Western Australia.

  How to register with HBF

  Provider Registration Form

• Dental Prosthetists

  To be eligible for HBF Registration as a Dental Prosthetist, you must practice solely and exclusively in private practice, as stated in HBF’s 'Fund Rules' (reference: e3.7)

  Please note

  You will need to complete the application form below and return it to HBF by mail - GPO Box C101, Perth, WA 6809 - or fax it to (08) 9265 6282.

  You will also need to provide the following:

  • A copy of your Health Department Licence

  or

  • If you are registered to use electronic claiming (HICAPS/iSoft) you will be registered as an HBF provider.

  HBF dental arrangements*

  HBF’s participating dental arrangements are designed to improve dental benefits for your HBF patients. As a participating dental provider, you continue to charge HBF members fees up to your individual fee schedule agreed with HBF.

  To be eligible for HBF Registration as a Homoeopathy Provider, you must practice solely and exclusively in private practice, as stated in HBF’s 'Fund Rules' (reference: e3.7)

  Please note

  To be eligible for HBF Registration as a Naturopath Provider, you must practice solely and exclusively in private practice, as stated in HBF’s 'Fund Rules' (reference: e3.7)

  To be eligible for HBF Registration as an Optometrist or Optical dispenser, you must practice solely and exclusively in private practice, as stated in HBF’s 'Fund Rules' (reference: e3.7)

  You will need to provide a copy of your registration with Medicare Australia, for each location you wish to register with HBF.

  If you are registered to use electronic claiming (HICAPS/iSoft) you will automatically be registered as an HBF provider.

  Optical Dispensers

  If you are an Optical Dispenser, you will need to complete an application form.

  To be eligible for HBF Registration as a Physiotherapist, you must practice solely and exclusively in private practice, as stated in HBF’s 'Fund Rules' (reference: e3.7)

  Please note

  You will need to provide a copy of your registration with Medicare Australia, for each location you wish to register with HBF. If you are registered to use electronic claiming (HICAPS/iSoft) however, you will automatically be registered as an HBF provider.

• Podiatrists

• Traditional Chinese Medicine

  To be eligible for HBF Registration as a Traditional Chinese Medicine Provider, you must practice solely and exclusively in private practice, as stated in HBF’s 'Fund Rules' (reference: e3.7)

  How to register with HBF

  Provider Registration Form

• Medical Provider

  This section of our provider registration site contains information for medical practitioners.

  It is not a requirement for medical practioners to be registered with HBF. The benefits for GPs or Specialists who register as an HBF provider are that they are able to submit patients’ invoices directly to HBF using the Simplified Billing system, removing the need for HBF members to claim benefits manually.

  This service is available to medical practitioners Australia-wide.

  Please ensure you read the appropriate Medical Gap Provider Guide featured below before signing the Express Pay Form. The Guides contain important information on using Simplified Billing.

  If you require a copy of HBF’s Schedule of Benefits, please indicate this on your email or fax when you submit your application for registration.

• Exercise Physiologists

  To be eligible for HBF registration as an Exercise Physiologist, you must practice solely and exclusively in private practice, as stated in our 'Fund Rules' (reference: e3.7)

  You will also need to provide the following:

  • A copy of current Professional Indemnity Insurance
  • A copy of current First Aid Certificate

  To be eligible for HBF registration as a Pilates Instructor (non-Physiotherapist) you must practice solely and exclusively in private practice, as stated in our 'Fund Rules' (reference: e3.7)

  • A copy of Current Professional Indemnity Insurance (as a Pilates Instructor not a Fitness Instructor)
  • A copy of current CPR/First Aid
  • A copy of completion of a tertiary level Pilates teacher training program with a minimum of 100 hours
  • A copy of completion of supervised clinical practice with a minimum of 100 hours
  • Evidence of current registration with the Physiotherapist’s Registration Board of WA

• Specialist Psychologists

  To be eligible for HBF registration as a Psychologist, you must practice solely and exclusively in private practice, as stated in our 'Fund Rules' (reference: e3.7)

  Please note

  You will need to complete the application form below and return it to HBF by mail - GPO Box C101, Perth, WA 6809 - or fax it to (08) 9265 6282.

  You will also need to provide the following:

  • A copy of current Professional Indemnity Insurance
  • A copy of current First Aid Certificate

  plus

  Evidence of registration with the WA Psychologist’s Registration Board and granted an approved Specialist title by the Board.

  If you are practicing outside of WA

  The current eligibility criteria for HBF Registration for Psychologists is:

  • A copy of full member of a Specialist College of Australian Psychological Society.

  Provider Registration Form

• Yoga Instructors

  To be eligible for HBF registration as a Yoga Instructor, you must practice solely and exclusively in a private practice, as stated in our 'Fund Rules' (reference: e3.7)

  Please note

  You will need to complete the application form below and return it to HBF by mail - GPO Box C101, Perth, WA 6809 - or fax it to (08) 9265 6282.

  You will also need to provide the following:

  • A copy of current Professional Indemnity Insurance
  • A copy of current First Aid Certificate

  plus

  Copies of an Australian accredited qualification in one of the following:

  • Advanced Diploma of Yoga Teaching
  • A course that includes a minimum of 320 hours yoga teacher training/experience

  or

  Copies of current membership to one of the following Professional Associations:

  • Yoga Teachers Association of Australia (YTAA) - Level 2 or 3 Recognised Member
  • Friends in Yoga (FinY) - Level 2 or 3 Recognised Member
  • BKS Iyengar Yoga Association of Australia - Iyengar certified teacher
