While working with your GP is essential for good health, the most important person in managing your health is you. The Coach Program complements care from your usual providers with telephone health coaching suited to your needs, so you can take more control of achieving better health outcomes in the long term.
We understand life gets busy, which is why the program is designed to fit around your other priorities. We do our best to accommodate your preferred days and times and can even provide after-hours phone coaching. The Coach Program could even save you time by helping you understand how to get the most out of your GP and specialist consultations, as well as manage your condition more effectively.
By transforming factors that put you at risk into protective factors, you can improve your heart health and understand the influence that medications, nutrition, activity, lifestyle behaviours and regular blood monitoring can have on heart health.
Preventive measures shouldn’t stop after a procedure such as a bypass or stent as subsequent heart events and procedures are common. Bringing your risk factors such as cholesterol and blood pressure to within recommended targets is even more important after a procedure and The Coach Program provides proven and practical advice to assist.
