HBF has an in-house team of experts, and partnerships with external providers that offer education, coaching and access to care to help manage chronic conditions like heart disease, diabetes, asthma and sleep apnoea.

The COACH program
Health Support Program - Get paired with an experienced health coach who’ll take you through a six-month program with a personalised plan to help you to achieve your health goals. With your health coach you’ll discuss clinical and lifestyle recommendations based on your health condition so you can translate these into action.

Healthy Living Program - Get memberships with leading health organisations such as Diabetes Australia, Coeliac Australia and Parkinsons WA. Membership benefits can include information, online communities and access to expert advice.

How do I know if I’m eligible for a program?
Eligibility for health programs depends on your HBF cover. Some programs are included in Hospital cover, and some programs are included in Extras cover.
Eligibility may also depend on program-specific clinical criteria. For example, you may need to have an official diagnosis for a condition, or you may need to have a certain body mass index (BMI) to be eligible for certain health programs.
