Who is eligible for The COACH Program?
To be eligible you must:
- Be 18 years or over
- Hold an eligible hospital product
- Currently be managing or at risk of a chronic health condition (including conditions like prediabetes, type 1 diabetes, type 2 diabetes, cardiovascular disease, chronic obstructive pulmonary disease, and peripheral vascular disease)
- Additional eligibility criteria may apply

Why does HBF offer The COACH Program?
HBF are committed to supporting the health and wellbeing of our members. The COACH Program has been designed to inform and support you to proactively manage your health and achieve the best possible health outcomes.
My doctor/specialist is taking good care of me, how will The COACH Program benefit me?
Working with your GP is essential for good health management, but you have the most important role to play in your health care. We aim to support you and your health goals by complementing your existing care with additional information and coaching.
I have a busy schedule, how will I fit The COACH Program into my week?
We are mindful of how busy life can get. A phone-based service eliminates travel time and the monthly appointments can be scheduled for your convenience. We are available Monday to Friday 8am-5pm and after-hours appointments can be accommodated upon request.
Does participating in The COACH Program have any impact on my health policy premiums or benefits?
No. As the holder of an eligible hospital policy The COACH Program is available at no additional cost by HBF as a way to help you manage your health and wellbeing. Participating does not impact the premiums you pay or the benefits you are entitled to under your health cover.
