What are HBF Health Support Programs?
HBF has an in-house team of experts and partnerships with external providers that offer programs supporting chronic health condition management.
These Health Support Programs focus on conditions such as osteoarthritis, cardiovascular disease and diabetes.
Programs we offer
Osteoarthritis Healthy Weight for Life

A structured 18-week weight loss program designed to help members improve knee and hip osteoarthritis symptoms, incorporating meal replacements, exercise guidance, pain management strategies, and health education and support.

The COACH Program

A six-month telephone-based health coaching program designed to help members manage chronic health conditions such as cardiovascular disease, diabetes, and chronic obstructive pulmonary disease (COPD).

The GLA:D program

In-clinic group exercise and education sessions with a trained physiotherapist, to help reduce knee and hip osteoarthritis symptoms.

Stand up to Joint Pain

An individualised and physiotherapist led exercise plan delivered by telehealth to help members manage symptoms of knee and hip osteoarthritis.

Frequently asked questions
My patient is considering participating in one of HBF's health support programs. Will HBF keep me informed on my patient's progress?
The intention of all HBF health support programs is to complement the care provided by a member's usual health care team. We encourage participants to nominate a healthcare provider to receive communications and, with an individual participant's consent, we will communicate with healthcare teams to keep them informed of an individual's progress and outcomes.
What is the cost for participating in an HBF health support program?
The COACH Program, Osteoarthritis Healthy Weight for Life, and GLA:D are fully covered for members who hold eligible HBF hospital cover and meet the programs’ eligibility criteria. See individual program pages for more information on eligibility criteria.
